Confirmation Registration Instructions

Registration should be filled out by parent and child together. Once you begin the form you cannot save it, if you exit out you will have to restart.

You will need the following information to register:

• Date & location of candidate’s Baptism [copy of certificate is due at orientation]
• Date & location of candidate’s First Communion
• Contact information for Candidate's Sponsor [sponsor forms are due at orientation]
• Credit/debit card for online payment [if payment poses any issue, contact Mrs. Catherine]

To better accommodate the different schedules of our students, in both fall and spring sessions, our small groups happen in 2 shifts on Sunday afternoons instead of all at once. Each shift begins with Mass, includes lunch or dinner, and ends with adoration.  
The two small group shifts will be:
   11:30am-2:30pm & 6:00-9:00pm
Unless you have a strong need to do one shift over another, please be flexible and choose "I have no preference". This will help us in determining small groups.
