  Complete this form after using the Wedding Liturgy Practice Worksheet to prayerfully plan your wedding liturgy.
  Submit your completed information no later than 8 weeks prior to your wedding.
  Any changes to music selections or readings after submission must be approved by the Director of Music and Liturgy.
  Your St. George Wedding Coordinator will contact you no later than 2 weeks prior to your wedding to review final details.
  Any changes to your bridal party information should be communicated to your Wedding Coordinator at that time.
